Clinical History

This 47-year-old woman was admitted with terminal carcinomatosis. On examination, a hard liver and a right pelvic mass were palpable. She had been experiencing constitutional symptoms for months and finally sought medical attention. Admitted for palliative care, she passed away shortly afterward.

Pathology

The left lung has been longitudinally sliced to reveal the cut surface. Numerous pale tumor nodules of varying sizes are scattered throughout the lung substance. Some nodules near the hilum are confluent, and the hilar lymph nodes contain pale tumor tissue. Small tumor nodules are also observed beneath the thickened pleura. Histologically, these are identified as metastatic deposits of adenocarcinoma. Post-mortem examination revealed adenocarcinoma of the ovary, with metastases in the lungs, heart, liver, and pericardium.

Pulmonary metastases are more commonly encountered than primary lung cancer. Malignant diseases originating from any part of the body may spread to the lungs. Sarcomas typically metastasize through the bloodstream, while carcinomas can spread via the bloodstream, the lymphatic system, or both.
